Stella Maris Community Based Services includes personal care services, skilled home health and hospice care. CBS services are designed to meet patient needs in the comfort of their home, whether their home is in the community, senior living, or long term care. Personal care services provide one on one support for clients who may need help with ADL’s, meal prep, or an escort to the doctor. Skilled Home Health provides physical therapy, occupational therapy, speech therapy, skilled nursing, and social work services to those who have experienced a change in their medical condition or health. For 40 years, Stella Maris has provided hospice services to the community. Whether in their home or on the inpatient unit, Stella Maris hospice providers help with care and comfort of our patients and their families. As the Community Liaison Manager, I oversee the outreach and education efforts for all of the Community Based Services.
